Hello and welcome to Lifeline Medical Center. Below, we have included all necessary information about our offices, and scheduling with our offices. Please review this information carefully, as it is crucial to ensuring that your appointment is scheduled and completed smoothly.
Lifeline Medical Center is a Mental Health Company offering Therapy and Psychiatry services in Pennsylvania (both telehealth and in-person), Oregon (only telehealth), and Michigan (only telehealth). All Telehealth sessions scheduled with the providers at our offices are completed through Therapy Portal.
Upon requesting an appointment with a provider at Lifeline Medical Center, one of our schedulers at our office will contact you. This call can take anywhere from 5 to 15 minutes. Upon speaking with you, the scheduler will first confirm the availability of the appointment you requested. Our scheduling team will promptly notify you if an appointment request needs to be adjusted and will secure the first available appointment.
Once your appointment is confirmed, the scheduler assisting you will provide you access to the portal and guide you through the necessary steps to complete any required documentation as well as how to complete your appointment with the provider when it comes time to doing so. Additionally, you will receive an email containing all the information provided during the phone consultation, as well as our main office phone number, the link to our portal, the scheduler’s phone number, and the scheduler’s email address in case of any further assistance.
If you do not receive the email and require assistance, please contact our office at 814-408-0014. If our scheduling team is not available, please leave a voicemail, and we will get back to you as soon as possible.
Upon scheduling your appointment, kindly upload a photo ID to verify your residency and personal identification. Additionally, please upload your insurance card to avoid out-of-pocket expenses. Upon requesting your appointment, you will receive a notification from Zocdoc indicating that two documents will need to be completed. You may need to close Zocdoc and return to your appointment request to view these documents. We kindly request that you complete these documents as soon as possible. These documents are identical to those you would complete if you were being seen in the office. The first document you will need to complete is the intake form, which will include your name, date of birth, Social Security number, emergency contact information, and other relevant details. The second document you will need to complete is the history form, which provides the Therapist and/or Psychiatrist with comprehensive information about your mental health history and needs. These documents are crucial and must be completed within 24 hours before your scheduled intake appointment. Failure to complete these documents may result in the rescheduling of your appointment.

She was very sweet and than I was told I had to talk to someone else and that the person she was having me talk to was great which I kinda was unsure why I had to talk to another person to begin with, I understand there are laws and regulations with some meds, than the first 3 mins of the person she referred me to immediately told me she couldn’t do anything. It was a complete waste of time, and I’m extremely upset about this.

She cancelled even though I told them I had a deadline at my school.
She then rescheduled and cancelled that too. Wish i would have read the reviews before going with her.

There aren't many providers of color that are women, so I really appreciated the representation. Being able to talk to someone that looked like me was refreshing, then getting down to the actual session she is very patient. I know it's her job but she actually seems like she cares. I didn't have to repeat anything, she kept pace with me the entire time and suggested what I should do next rather than ask. Which I also appreciate.
