Lily Health and Wellness Center provides compassionate, client-centered care focused on supporting mental health and overall well-being. Our practice offers a safe and welcoming environment where individuals can receive personalized support for emotional, behavioral, and life challenges. We are committed to helping clients build resilience, improve coping skills, and achieve healthier, more balanced lives through evidence-based approaches and holistic care.
Please complete all required intake forms and add a credit card on file to keep your scheduled appointment. After scheduling, you will receive an email from no-reply@icanotes.com with instructions to set up your secure client portal and complete the required paperwork. For the best experience and to avoid technical issues, we recommend completing the forms using a laptop or desktop computer rather than a mobile phone.
The intake paperwork collects important information such as your contact details, medical and mental health history, current medications, and required consent forms. Completing these forms before your visit allows our clinical team to review your information ahead of time and prepare for your appointment so we can focus on your care during the visit.
A valid credit card must also be added to your file through the client portal. This is required by clinic policy to reserve your appointment time. If the intake forms are not completed and a credit card is not placed on file, the appointment will be canceled within one hour.
Please also be aware that missed appointments or cancellations without prior notice prevent other patients from receiving care. Because of this, a no-show or late cancellation will result in an $80 fee.

Dr. Similade is very patient and is a very good listener. She offers pragmatic solutions to the problems you may be having. It’s weird because she helps to shift your perspective from being defeated to building you up. I can do hard things. I can do this.

She was friendly and seemed like she was listening but not a good fit. The call lasted about 30 minutes, unhelpful in providing the guidance I was looking for on my journey. She asked if I had life insurance and what type of policy I had. Not sure if that’s part of the normal process but it seemed unnecessary.

Initially “Dr. Ade” seemed oddly distant, but that was probably because I submitted my intake form at the last minute. It was clear she was not pleased. However, as the visit progressed, she became warm & caring, and made me want to open up to her. And it’s clear she knows medicine.

Very professional, I've just moved to Maryland and was unsure of what I would do about my medications. I found this doctor and she really listened to me, she understood my concerns. I recommend her highly because I'm used to doctors just forcing me from medication to another but she didn't. This is a doctor that cares about your mental health and well-being.
