Please be aware, MB Wellness Associates does not provide any controlled-substance prescriptions or medication refills until the second visit.
Patients requesting a controlled-substance prescription must come to the office for an in-person visit prior to receiving a prescription.
We are unable to complete any legal documentation (FMLA, Emotional Support Letters for Animal, Etc.) within the first 3 months of visit.
No show fee up to $100 may be charged if the appointment is no cancelled or the provider notified with in 6-12 hours of the appointment.

She didn’t listen to what I had to say what other doctors nurses and everyone has been suggesting to me that would work for me. Instead, she took control like she was the master of everything didn’t care about my feelings what I’m going through mentally and she laughed when I said I did not like her decision. Everyone has the right to say what they want to say, but you don’t laugh in someone’s face

Delayed my appointment by a half hour, showed up a smidge late (that particularly wasn't a big deal), her equipment didnt work for 15 mins, and she wanted to do a full screening including a physical for a mental diagnosis from a good psychiatrist I've already had for 6 years.
