Miami Wellness and Aesthetics Center
We are a Family Medicine practice that also specializes in cosemetic injectables such as Botox, Dysport, Fillers (Restylane, Juvederm), & natural biostimulators such as Sculptra and PRF EZ Gel.
Please understand it is YOUR RESPONSIBILITY to make sure we are in network with your insurance prior to booking if you are expecting your visit to be covered. If we are out of network with your insurance or if you are coming in as a self-pay patient, you will be charged $300 for a new patient visit and $200 for follow up visits thereafter.
We accept most Cigna, Blue Cross/Blue Shield, Aetna, and Oscar PPO insurance plans. However, please be advised that if you have a deductible with your insurance that has not been met, we do collect an office visit fee of up to $300 at the time of your visit.
If your insurance plan covers an Annual Wellness Exam, please read a description of what an Annual Wellness Exam entails:
The Annual Wellness Visit (AWV) is a yearly appointment with your primary care provider (PCP) to create or update a personalized prevention plan. This plan may help prevent illness based on your current health and risk factors. Keep in mind that the AWV is NOT a head-to-toe physical exam.
***Anything outside of preventative care (diagnostic) is subject to a copay or your deductible***
These areas ARE NOT covered by your insurance during your annual wellness visit:
Sick visits: If your time with your doctor involves a discussion of an illness or symptoms, from an infection to abdominal pain, and your doctor recommends care or a treatment plan, you will be charged a copay or a deductible.
Chronic illnesses. Your insurance for your annual wellness visit does not cover any discussion, treatment or prescription of medications for chronic illnesses or conditions, such as high blood pressure, high cholesterol or diabetes. In this instance as well, you will be charged a copay and/or a deductible.
In advance of your appointment, we encourage you to consult with your insurance provider if you have questions or concerns about your coverage.
What’s the difference between preventive care and diagnostic care?
Diagnostic care is related to services in which your provider is looking for something specific, often based on the results of a preventive test or screening. For example, your PCP may ask for a follow-up appointment for a patient based on abnormal lab results. This follow-up is to check for something that may have been detected during the preventive or routine exam. The follow-up appointment is diagnostic, and NOT COVERED AS PREVENTATIVE CARE.
