WELCOME TO OUR PRACTICE We’re glad you’re here today. We are grateful you have selected our office to provide your child’s dental care and look forward to you becoming a long term part of our practice. Our commitment to our patients is to provide the highest quality of service in a comfortable, caring and nurturing environment. SOME THINGS WE WOULD LIKE YOU TO KNOW We appreciate payment at the time of service. We cannot provide services on the assumption that our fees will be paid by an insurance company. All services are charged directly to you, and you are personally responsible for payment. As a courtesy to you we will prepare any necessary reports and itemized breakdowns to assist you in getting reimbursed for our services by your insurance company. This makes it possible to continue to provide our high level of care at a reasonable cost. For your convenience we accept VISA, MASTERCARD, DISCOVER, and AMERICAN EXPRESS. Generally, if you use a credit card for payment you should receive your insurance reimbursement before you even have to pay the credit card bill. This results in no money actually out of pocket for you. NOTE: In cases of divorced parents, if the court has awarded custody to one parent and the financial responsibility to the other parent, or if both parents are jointly responsible- the parent bringing the child to us is responsible for payment in full. We will provide any information necessary to inform the child’s other parent.
