There are some intake forms and a questionnaire that must be completed before we can confirm your appointment. Our team will verify your insurance so that you know what your out of pocket cost will be (if any). Dr. Elhalaby was a fantastic person to meet and speak with about my diagnostic and prescriptive needs. He didn't rush me to speak, was attentive, and asked plenty of follow-up questions that helped me get to the core of my concerns. Highly recommended, I will be working with him for as long as I can.
