We are fully licensed and able to practice and able to see patients in Arizona, Illinois, Iowa, Kansas, Montana, Nebraska, North Dakota, Utah, and Washington.

- there is a $75 cancellation fee for appointments cancelled with less than a 24-hour notice, this fee applies to same-day appointments too.
- Due to outdated systems at Walmart, our practice does not send prescriptions to Walmart pharmacies.
- We do NOT see patients with Medicare or any Medicare Supplemental insurance.
