Personal Concierge MD
I want to welcome you to Personal Concierge MD, a boutique internal medicine practice.
If you are having a medical emergency such as chest pain, shortness of breath, severe abdominal pain, numbness, tingling or other stroke-like symptoms, auditory hallucinations, visual hallucinations, suicidal, homicidal thoughts, please call 911.
If you are having a cough, fever, body aches, ear aches, sinus symptoms, diarrhea or any other symptoms related to Covid-19, the office is only accepting tele-health visits for evaluation.
This practice does not manage acute medical situations in office including blood pressures higher than 160/110. Please go to the nearest urgent care or emergency room for care.
Please be advised that our doctor ONLY practices Adult Care. The doctor does not practice psychiatry, pain management, geriatrics or pediatrics.
Our office accepts the following insurance plans: Cigna, Anthem, Blue Cross Blue Shield, Aetna, Humana and UnitedHealthcare.
Unfortunately, the practice does not accept any insurance plans affiliated with CareSource, WellCare, First Health Network, Oscar or Freedom Life Insurance Company of America
Please be advised that self-pay, copay, deductible and coinsurance
payments are expected to be paid at the time of service.
Please complete the electronic forms and assessment that will be sent to you by a text or email from our Epion program partner.
Please confirm your appointment via text with our Holly AI program.
Non-membership appointments will be provided by our nurse practitoner.

Recently I searched for a new doctor for my adult health and wellness. I was needing something *different* - a personal & closer doctor / patient relationship.
So far so good!
Before my 1st appointment, I completed the digital medical history online. Although this did take some time, it allowed me to have more 'productive' time with Dr. Hayes during the visit.
When I arrived, Dr. Hayes immediately greeted me. We reviewed my history, he asked questions, conducted his exam and gave me a comprehensive wellness check/feedback.
Shortly after leaving the appointment, my online portal contained all of his notes, referrals, and details from my appointment. Having that thorough information available was helpful and appreciated.
Since then, I've sent him messages through the portal and he has responded. Direct communication with the doctor is something new, different. He even reminded me in one message about another item that was unrelated - signaling to me that he's always looking at the whole picture.

I would recommend Dr. Hayes for male and female prospective patients of all ages, but as a 40 year old man, he is an excellent resource that can completely guide me through things to focus on. This is perfect for busy people that want 1:1 help at any time. The office is also pristine, which is a nice plus.
