She has great experience in providing primary care and advanced primary care services to the diverse population of South Florida. Dr. Siram is an enthusiastic, compassionate physician and is committed to provide excellent care to her patients. Primary care 369 is dedicated to provide personalized approach in treating various disease conditions of her clients and enhancing health. She would like to guide using a Comprehensive approach to obtain great results for an ultimate well being."

Annual Wellness Visit can be done only by in-person in the clinic and cannot be done virtual. Do not take appointments for Annual Wellness Visit as televisit/virtual visit.
We also offer following services-
Shockwave therapy for Arthritis, low back pain, Tendinitis. Shock wave therapy helps in boosting the regenerative power of tissues and modulate restructuring.
Weight Loss program with education on nutrition, lifestyle modifications and treatment with Semaglutide, Tirzepatide, phentermine, contrave.
Vitamin B 12 injections are available for self pay price of $25.00
Pricing for above services are self pay priced. Call us before making appointment.
We collect partial fee of $85.00 for deductibles plans up front on day of service.
For self pay patients, $175.00 will be collected on day of service.

Prior to appointment I did receive a phone call to take care of some initial paperwork for information which I'd prefer to have done and not have to delay my appointment to do so when I arrive. I was attended quickly and eith no issue. First time here and will be appointing as my PCP.

She was a great listener, she took time to actually hear my concerns, wasn’t judgemental or pushy, encouraged me to be a part of my own healthcare decisions, and was open and honest about risks and benefits of each choice. At the end of my appointment she developed a plan WITH ME.
