Please note that I use a website called Headway to do my insurance billing. So, after you book an appointment, I will email you an invite from Headway for you to complete their paperwork, which is just your insurance info, address, etc. I ask that you also complete this paperwork prior to your visit. It should only take a few minutes; they just need images of your insurance card and demographic information. Please note that if you are not located in CA, AZ, or HI, then even if Zocdoc allows you to book with me, your insurance will not cover the visit and you will be responsible to pay, prior to the visit.
