-NO CALL/NO SHOWS/LATE CANCELLATIONS will be billed a $75 fee per office policy.

*REFERRALS FROM YOUR PRIMARY CARE DOCTOR ARE MANDATORY if you have an HMO for insurance (unless your card specifies otherwise)
-To prevent delays in your care, all intake forms must be completed prior to your appointment time, including a picture of your ID/drivers license, insurance card and a completed credit card authorization form.
-All CANCELLATIONS must be made 24 hours BEFORE your actual appointment time to prevent the mandatory LATE CANCELLATION FEE.
S.P. Women's Health NP Services, PLLC offers a comfortable and caring environment where you can address all of your personal needs and concerns. Our C.E.O. and founder S. Perry, is a Board Certified Women's Healthcare Nurse Practitioner who takes pride in the care she delivers with a special focus on patient satisfaction. She has over a decade of clinical experience in emergency medicine and women's healthcare, including a certification as a Sexual Assault Forensic Examiner (SAFE) Registered Nurse. Ms. Perry completed her undergraduate studies at The Pennsylvania State University (State College, PA) and obtained her graduate degree from The SUNY Downstate Medical Center (Brooklyn, NY). She is not only the C.E.O. of our practice, but she also works for clinics in underserved communities and at NYU Langone Medical Center.
Ms. Perry believes that all women should have the opportunity to be properly and thoroughly educated on the fundamentals of their bodies. Additionally, with patience and compassion, she strives to provide a holistic experience that transcends medical boundaries.
