BEGINNING JANUARY 2024: NEW PATIENTS CAN CHOOSE MEMBERSHIP PLANS WITH/WITHOUT INSURANCE. IN THE ABSENCE OF MEMBERSHIP,ANY INSURANCE FILING ON BEHALF OF THE PATIENT WILL BE AT A CHARGE of 100 dollars per year.
IF you need refills on previous prescriptions on your first visit, please bring records of original prescription and/or Medical records from previous provider with reason for medication.
After hours/REMOTE CARE ACCESS and VIDEO VISITS appointments available for established patients for an ANNUAL FEE.
Please bring your COVID vaccination information and if symptomatic and unvaccinated , ensure negative COVID test prior to arrival.
NO COVID TESTING DONE AT THE OFFICE. PLEASE WEAR YOUR MASKS.
