Seraphim Mental Wholeness, LLC
We help clients dealing with issues like ADD/ADHD, anxiety, depression, OCD, mood disorders, panic, and more. To make the process easier, please upload the front and back of your insurance card along with your ID card, and provide the necessary information. We will have you enter your credit card information with our billing company. Please cancel your Zocdoc bookings if you know you are not ready to comply, or do not agree with entering a valid credit card info. We do not begin treatment with controlled medications, especially if you are not already taking them. Treatment is personalized, not a one-size-fits-all. If you complete the required paperwork on time, we can see you the same day. Booking us on Zocdoc does not mean appointment is confirmed.
