INSURANCE & IDENTITY VERIFICATION We accept the insurances listed on each therapist's profile. To verify your identity and insurance coverage, please upload the following documents directly in ZocDoc: • Front and back of your insurance card • A valid photo ID (or a parent/guardian's ID for minors) We will share all information provided to us by your insurance carrier. REDUCED/EQUITABLE FEE OPTION If you are paying out of pocket and would like to apply for a reduced rate, please email us at info@torresmhcs.com after scheduling. Reduced session fees are available to those who are uninsured, have coverage we are not in-network with, or are choosing not to use their insurance. BILLING *** Our system requires a debit or credit card on file for automatic billing. Standard Fees: • Intake — $225 • 60-Minute Session / Couples Counseling — $200 • 45-Minute Session — $175
