**Medicaid requires one week pre-authorization before booking** **We are an out of network UHC and BCBS provider, co-insurance applies** **Call your insurance and confirm they accept code 97802** We assist with but are not limited to, Diabetes, Pre-Diabetes, Weight Loss, Oncology, Post-Surgical Healing, Pre-Surgical Nutrition, Gut Gealth, Public Speaking, Support Groups, Home Visits, Lunch-and-Learns, and Corporate Wellness. Our discovery call will help you understand where we can help and how we will do it. At Vital Nutrition, we recognize that achieving optimal nutrition and well-being can sometimes feel like an uphill battle. We understand the frustrations, confusion, and conflicting information that can surround the world of nutrition. Vital Nutrition will provider a superbill when requested for clients to pursue insurance reimbursement. Currently in-network with Medicare A/B for CKD and DIABETES TYPE II Form of payment must be on file prior to appointment.
