We're looking to build long-term treatment relationships with patients committed to their care plan! After you Book your Time Slot: After submitting this form, please call our office at 540-254-7899 to complete your registration in our Charm EHR patient portal. Additional required consents are completed through that portal and must be finished 24 hours before your appointment. What should I expect for my first appointment? New patients complete a 60-minute intake evaluation. Please complete your intake paperwork as soon as possible after booking, and no later than 24 hours before your visit. A credit card on file is required for all new patients, and a $200 no-show fee applies to missed or late-cancelled first appointments (less than 24 hours' notice).
