Before Your Appointment As part of preparing for your visit, you will receive separate emails and text messages from our electronic health record system, PracticeQ. Please check your spam or junk folder if you don't see them. Within those communications, you'll find an invitation to: • Sign up for the Patient Portal — where you can securely access your health information, send messages, and review your visit details. • Complete your intake paperwork — it's important that all forms are filled out prior to your appointment. This saves both your time and Dr. Opelami's, ensures she has what she needs to prepare for your visit, and allows for a smoother, more meaningful experience together. Completion of intake paperwork is required prior to your appointment. VTR Health Is a Self-Pay Practice VTR Health does not accept or bill insurance. We are a direct-pay (self-pay) practice, which means you pay VTR Health directly for your visit rather than going through an insurance company. This lets Dr. Opelami spend your appointment on your care instead of on prior authorizations and claims paperwork, and it keeps our pricing simple and transparent. • If you have an HSA or FSA account, you can typically use it to pay for your visit — check with your plan administrator. • If you'd like a receipt to submit to your insurance for possible out-of-network reimbursement, we're happy to provide one — VTR Health does not file claims on your behalf, but the receipt is yours to use as you see fit. Payment Is Required Before Your Appointment Payment is due in full at the time of booking or before your visit begins. Your appointment will not be confirmed or started until payment has been received. We accept major credit cards, debit cards, and HSA/FSA cards. Our Pricing Below is a summary of current VTR Health pricing: Urgent Care Virtual urgent care visit — evaluation, diagnosis, prescription and lab orders as needed $80 per visit, due at booking. Weight Management (GLP-1 / GIP Program) Initial Evaluation (one-time) — 60-min physician consult, full history and contraindication screen, baseline labs ordered, personalized medication plan $179, due before your visit. Medical Management (monthly) — scheduled visits per protocol, standard WhatsApp response $99/month. Complete Care (monthly) — more frequent monitoring visits during titration, priority WhatsApp response $179/month. Medication is never bundled or marked up by VTR Health, in either tier — you pay the pharmacy directly for your medication, so your VTR Health program fee stays fixed regardless of pharmacy pricing changes. Primary Care Primary care membership and chronic disease management $100/month subscription, billed monthly Cancellations & Rescheduling I understand that life happens and schedules can change. If you need to cancel or reschedule, please let us know as early as possible or rebook directly at vtrhealth.intakeq.com/booking. • Cancellations or reschedules made within 24 hours of your appointment are subject to a no-show fee of 50% of the visit cost.
