We welcome patients from Mamou, Oakdale and neighboring communities, including Ville Platte, Eunice, Basile, Church Point, Pine Prairie, Opelousas, and rural areas of Evangeline, Allen, Acadia, and St. Landry Parishes. Our Providers Upendra Kulkarni, M.D., F.A.C.O.G. is a board-certified OBGYN with decades of experience caring for women with routine and more complex obstetric and gynecologic needs. His postgraduate training in New York included one year of pediatric residency followed by four years of Obstetrics and Gynecology residency, where he served as Chief Resident during his final year. Katie Deshotel, APRN, WHNP, is a Women’s Health Nurse Practitioner who has been with the practice since 2018. Born and raised in Eunice, Louisiana, Katie provides approachable, compassionate women’s health care for adolescents and adults and has a special interest in helping teens and younger women feel comfortable discussing their health. Women’s Health Care Services may include annual & well-woman visits; Pap and cervical cancer screening when appropriate; contraception and family planning; Nexplanon insertion and removal; evaluation of irregular, painful, heavy, or missed periods; PCOS and hormonal concerns; abnormal uterine bleeding; postmenopausal bleeding; pelvic pain; endometriosis; STI evaluation and treatment; urinary concerns; breast concerns; perimenopause; menopause; and postmenopausal care. Treatment is individualized based on symptoms, history, risk factors, preferences, and clinical circumstances. Adolescent & Teen Women’s Health We welcome adolescents and teens for age-appropriate women’s health care. A first women’s health visit does not automatically mean a pelvic examination or Pap test. Many visits begin with conversation, education, menstrual or developmental concerns, and questions about health. Care is individualized according to age, symptoms, medical history, the reason for the visit, and applicable consent and privacy requirements. Pregnancy, Prenatal & Postpartum Care We provide prenatal care, delivery, & postpartum care, for routine and/or high-risk pregnancies. Care may include laboratory testing, genetic screening, ultrasound monitoring, nonstress testing, discussion of medications and symptoms, delivery planning, postpartum recovery, breastfeeding concerns, contraception, and coordination with specialists when appropriate. Dr. Kulkarni provides obstetric care and deliveries, and Katie participates in prenatal and postpartum care in collaboration with him. High-Risk Pregnancy Some pregnancies require closer monitoring because of maternal conditions, pregnancy complications, previous pregnancy history, fetal concerns, or other risk factors. Dr. Kulkarni has extensive experience caring for high-risk pregnancies. Monitoring is individualized and may change as pregnancy progresses. Infertility, PCOS & Hormonal Concerns Patients may come to us for difficulty becoming pregnant, irregular ovulation, Polycystic Ovary Syndrome (PCOS), menstrual changes, or other hormonal concerns. PCOS can affect menstrual cycles, ovulation, fertility, skin, hair growth, weight, and metabolic health. Not every patient with PCOS has ovarian cysts. Evaluation and treatment are individualized. Female Urinary Incontinence & Bladder Control Dr. Kulkarni has more than ten years of clinical experience evaluating and treating female urinary incontinence. Bladder leakage may occur with coughing, sneezing, laughing, exercise, lifting, or with a sudden urge to urinate. Treatment depends on the type and cause and may include conservative measures, pelvic-floor approaches, medication, office-based treatment, or surgery when clinically appropriate. Bladder leakage is common, but it does not have to simply be accepted as an inevitable part of childbirth, aging, or menopause. Pelvic Pain & Endometriosis Pelvic pain may have many causes, including endometriosis, ovarian cysts, uterine conditions, pelvic-floor problems, urinary or bladder disorders, gastrointestinal conditions, musculoskeletal problems, and others. Evaluation may include history, examination when appropriate, laboratory testing, imaging, and review of previous treatment. Recommendations depend on the suspected cause and the individual patient. Perimenopause, Menopause & Beyond Perimenopause and menopause can bring changes in bleeding patterns, sleep, mood, hot flashes, vaginal symptoms, urinary symptoms, and other concerns. Not every symptom during this stage is necessarily caused by menopause, and postmenopausal bleeding should be evaluated. Gynecologic Surgery Not every gynecologic condition requires surgery. When surgery is appropriate, Dr. Kulkarni considers minimally invasive laparoscopic or robotic-assisted approaches for appropriately selected patients. These approaches may offer benefits such as smaller incisions and shorter recovery for some patients. The appropriate procedure, benefits and risks, alternatives, and expected recovery vary by diagnosis and individual circumstances. A Teen’s First Women’s Health Visit A first women’s health visit can feel unfamiliar, especially for a teen or parent who is unsure what to expect. Many first visits focus on conversation, education, menstrual or developmental concerns, and questions about health. A pelvic examination or Pap test is not automatically required at a first visit. The visit is individualized according to age, symptoms, medical history, reason for the appointment, and applicable consent and privacy requirements. Irregular Periods, PCOS & Hormonal Changes Periods that are unusually irregular, very heavy, painful, or absent can have many causes. PCOS may affect menstrual cycles, ovulation, fertility, skin, hair growth, weight, and metabolic health. Not every patient with PCOS has ovarian cysts. Evaluation and treatment depend on the patient’s symptoms, age, medical history, goals, and other clinical factors. Birth Control & Family Planning Patients may discuss contraception, pregnancy prevention, family planning, and reproductive goals with our providers. Options vary according to age, medical history, preferences, health conditions, and individual circumstances. Services may include oral contraceptives and other methods as well as Nexplanon insertion and removal when appropriate. Pregnancy Care Close to Home We provide prenatal and postpartum care for patients in Oakdale and surrounding communities. Care may include prenatal visits, laboratory testing, genetic screening, ultrasound monitoring, nonstress testing, discussion of nutrition and medications, evaluation of symptoms, delivery planning, and postpartum recovery. Dr. Kulkarni provides obstetric care and deliveries, while Katie participates in prenatal and postpartum care in collaboration with him. When Pregnancy Needs Closer Monitoring Some pregnancies require additional monitoring because of maternal conditions, pregnancy complications, previous pregnancy history, fetal concerns, or other risk factors. Dr. Kulkarni provides high-risk pregnancy care and coordinates additional specialty evaluation when appropriate. Monitoring is individualized and may change during pregnancy. Postpartum Care The postpartum period can involve physical, hormonal, emotional, and practical changes. Visits may address recovery after delivery, bleeding, pain, blood pressure, mood screening, breastfeeding concerns, contraception, wound or perineal concerns, fatigue, and the transition back to preventive care. Some patients may need earlier or additional follow-up. Having Difficulty Becoming Pregnant? Infertility can have many causes and may involve one or both partners. We provide infertility evaluation and care planning, which may include review of menstrual and ovulation history, laboratory testing, imaging, and discussion of appropriate next steps. Referral for additional fertility services may be recommended when appropriate. When Pelvic Pain Doesn’t Go Away Persistent or recurring pelvic pain may have many causes, including endometriosis, ovarian cysts, uterine conditions, pelvic-floor problems, bladder or urinary conditions, gastrointestinal issues, and musculoskeletal problems. Evaluation and treatment depend on the suspected cause and the individual patient.
