Below are the policies implemented at our practice to maintain quality healthcare for our patients:
1. When scheduled for a routine physical, urgent problems or issues might not be addressed during the same visit. Per insurance guidelines, if a new or urgent problem is addressed, the patient may be billed separately for that concern in addition to the preventive physical billing codes. Your doctor may recommend rescheduling your routine physical if you have other pressing or complex problems.
2. All copays, self-pay fees, and past due balances are due at the time of service. We reserve the right to require patients to reschedule if copays or past due amounts are not paid at the time of service. Patient accounts with a past due balance over 90 days will be sent to a collection agency.
3. Proof of insurance and personal identification are required at each visit.
4. Patients requesting medication refills will need an in-person or telehealth visit, which will be charged to their insurance or to self-pay patients accordingly.

Horrible… first they switched my appointment time without notifying me. When I arrived the doctor wasn’t there then I was told it was supposed to be a virtual appointment and there was another person there that they did the same thing to. Then they told the other client that they needed to take his vitals for the appointment which they had already done and did not know it. I would not recommend this office to anyone. They are not organized at all
