Dr. Carlos J. Crudup is a Board Certified (2006) Family Practitioner in Chicago, Illinois, and grew up on the south side of the city, graduating from De LaSalle High School. He is a graduate of Morehouse College with a Bachelor of Science degree in Biology. He completed a Doctor of Medicine degree from Finch University of Health Sciences/The Chicago Medical School in 2001, and completed his preliminary internal medicine internship at UIC/Advocate Christ Hospital and his Family Practice Residency at Advocate Christ Hospital in 2004
Dr. Crudup is very active in his community and has devoted his time and expertise to various grassroots organizations. His philanthropic work includes volunteering with the Chicago Area Health and Medical Careers Program (CAHMCPs), which is an organization that is responsible for increasing the number of physicians, dentists and other health professionals from under-represented minority groups. Other community service work include volunteering with the Chicago Medical School/Homeless Healthcare program; serving as board member for The Provident Foundation, which pays tribute to Dr. Daniel Hale Williams; mentoring college aged students interested in attending medical or nursing school; and participating in MedFest, which is an annual event sponsored by the Advocate Medical Group and Special Olympics of Illinois that provides physical examinations to participating athletes.
Dr. Crudup is a tenured physician with the Advocate Medical Group. As a valued attending physician, he has received various awards for competence and professional excellence. In 2007 and 2008, he received the Honored Member Cambridge/Metropolitan Who's Who Recognition Award. In 2007, he received the Advocate Health Centers Highest Patient Satisfaction for Family Practice, and the Advocate Health Center Outstanding Clinical Practice Award. Recently, in 2010, Dr. Crudup was rated in the top five percent nationally for “patient satisfaction” and given the Certificate of Recognition from the National Committee for Quality Assurance (NCQA); and he received recognition from The American Heart and Stroke Association that awarded him the Quality Cardiovascular Assurance and Stroke Care Award that extends from July, 2010 to July, 2013.
Dr. Crudup (Hyde Park Family Medicine), will be joining Hyde Park Dermatology. He is a highly respected physician amongst his colleagues and has a personable and caring nature, which makes him a welcomed addition to the Hyde Park Dermatology family.

Dr. Crudup was great! While some doctors I've had in the past have used scolding or negative tones, Dr. Crudup was warm, supportive, and encouraging. I felt listened to, reassured, and satisfied that all of my questions were answered.
I also felt that the discussion of my history and current health were thorough, and that the appointment was much more of a conversation than just a doctor talking at me. I was treated with respect and friendliness, and I'm excited to use Dr. Crudup for future appointments.

Wonderful as usual. We had a great pediatrician before but switching to Dr. Crudup was one of the best decisions we have made for our family. It is so easy to book appointments, all the staff are exceptional and we are always in and put of their in a reasonable amount of time.

Dr. Crudup was very informative and thorough with my visit. I had my blood drawn, and he went over the entirety of the lab results with me and explained what each section meant to my health.
He is very easy to understand, and I felt very comfortable speaking to him. I had my first visit about two weeks ago, and I did not feel uncomfortable speaking to him about any personal issues.
I would definitely recommend Dr. Crudup!

My Daughter & I absolutely LOVE this Dr. After her pediatrician retired, we went through a few Dr's we didn't care for much, before finding Dr. Crudup. He has been by far our favorite Dr. ever! We don't ever feel rushed during the appt, and feel he takes his time listening to all concerns.

I'm so happy to have found Dr. Crudup! My daughter is very picky when it comes to Dr's (which she should be), and from her very first appt, she LOVED him!! He was so very thorough and asked alot of questions and showed concern!! I would gladly refer him to everyone I know!! :)

I really enjoyed meeting him for the first time and his he had very cordial staff. I went on a Saturday morning and they only had one nurse. I needed some blood drawn and I had to come back to the front as more patients were coming. I waited for about ten-fifteen minutes after my meeting with him to get back with the nurse so that my blood could be drawn. That would be the only downside.

I will recommend him to anyone of my family members or friends in need of a physician. Dr. Crudup is a very likeable physician with excellent bedside manners. He presents with a warm smile and does not rush the patient; he is a very good listener. He covers all points very well and asks if there are any other questions that he did not answer or that you wish to have answered.
