Unlike standard clinics, we use technology like smartwatches to monitor your vitals from home, allowing us to catch potential health issues before they become emergencies.

If you manage a chronic condition like hypertension or diabetes, we may provide you with a Samsung Galaxy Watch or similar device at no cost. It automatically tracks your vitals and securely sends the data to our clinical team. If your numbers are off, our nurses will alert you and call you to check in, adjust medications, or schedule a visit.
Do I have to be "tech-savvy" to be a patient here?Is technical proficiency necessary to be a patient here?
Not at all. Our devices are designed to be user-friendly and typically require minimal user interaction beyond wearing them. Our staff, known as Patient Navigators, will guide you through the entire process during your initial onboarding.

We provide comprehensive Family Medicine and Advanced Primary Care for all ages. Our core services include:
Routine Care: Annual physicals, wellness exams, and sick visits.
