Dr. David Maze was born in Elgin, Illinois. He attended Streamwood High School. He earned a Bachelor’s degree Cum Laude from Benedictine University (formerly IBC). He received an OD degree from Illinois College of Optometry in 2003. He completed a Vision Therapy and Rehabilitation residency program at the State University of New York State Optometric Center in Manhattan.
He is a member of the American Optometric Association, Illinois Optometric Assn, Cornea and Contact Lens Section of the American Optometric Assn, the Neuro-Optometric Rehabilitation Association and the Optometric Extension Program. Dr. Maze earned his Fellowship (Board Certification in Vision Therapy) in the College of Optometrists in Vision Development in 2007. He is also a Fellow in the American Academy of Optometry. He enjoys running and has run many road races including several marathons, and has completed several triathlons including an full distance Ironman. He currently resides in Woodridge with his wife Kristen and their son James.
