The administrative staff person who assisted me when I checked in said they didn't get the documents I filled online. She also said they never use the online documents from ZocDoc. I couldn't tell if she didn't know how to get the documents or just didn't care, she was extremely dismissive and unhelpful. I had to fill out the information on a paper form.
