OFFICE POLICY:
1. Controlled substance prescriptions for new patients are provided for in-person office visits only, after reviewing medical necessity (no telehealth refills).
2. The office will collect an upfront partial deductible ($120) for high deductible plans for all visits.
3. Patients need to complete office intake forms 45 minutes before the appointment (ID proof and insurance card are mandatory).
4. We are primary care providers, not specialists; we can only provide specialist referrals.

Nurse Practitioner Bommana was very helpful. As a black women I search for medical professionals who are listeners, empathetic, and proactive about continuously enhancing their knowledge. Best doctors appointment I’ve ever had. I wouldn’t describe Nurse HARITHA as friendly per se, (though I can tell she is with her staff) she means business, professional and very respectful but firm. I felt reassured that she knows what she’s doing, without all that overly friendly fluff, I didn’t feel rushed, she firmly called me out on my unhealthy habits without talking down on me. Explained everything, everything she was doing and expecting from and what my expectations of her should be. Very satisfied with my appointment, I couldn’t wait to leave so I can write this review.
