NOTE TO PATIENTS:
Annual physical exams are booked Monday, Wednesday and Friday at 9:00am, 10:00am, and 1:00pm. Tuesday and Thursday 9:00am and 10:00am, there are no appointments after 12:00pm on these days two days.
Please be aware that there is a $25 fee for missed appointments and a $35 fee for missed physical exams.
