I recommend you dont bother filling out your new patient forms online through zocdoc as i was told to fill them out again at the office. Even thouh that was a dissapointing i have to say it was worth the wait. The lady at the front desk was professional and Dr. Jacobson was amazing. He really listened to my concerns and even advised me of side affects my medications might have on me without me asking. He truly showed he cared about my well being. The nurse who took my vitals was also outstanding she made me feel very comfortorable, she was kind and professional. Glad i chose Dr. Jacobson.
