My Approach to Care I believe that retinal disease, more than almost any other area of medicine, rewards precision paired with patience. The retina is unforgiving of guesswork, but it's also remarkably responsive when problems are caught early and treated thoughtfully. My approach is built on three commitments: rigorous diagnosis before treatment, clear communication so patients understand what's happening to their vision and why, and continuity of care that doesn't end when a procedure does. I trained as both a physician and a scientist. My MD/PhD at the University of Rochester focused on the developmental biology of the retina, specifically how retinal cells differentiate, which gave me a foundation in how the retina is built before I ever learned how to fix it when it breaks. That scientific training still shapes how I evaluate patients today. I don't treat a scan or a symptom in isolation. I want to understand the underlying mechanism, because that's usually the difference between a treatment that manages a problem and one that resolves it. Training That Shaped My Standards I completed my ophthalmology residency at the University of Oklahoma's Dean McGee Eye Institute, one of the country's most respected training grounds for general and subspecialty ophthalmology. I then completed fellowship training in vitreoretinal surgery at the University of Iowa under Dr. Edwin Stone, a leading figure in inherited retinal disease and genetic eye disorders, where I served as a Heed Foundation Fellow. That fellowship gave me deep experience not just in the surgical volume that builds technical skill, but in the diagnostic complexity of rare and inherited retinal conditions, cases where the answer isn't obvious and the workup has to be thorough. Research-trained diagnostic rigor. My doctoral research on retinal cell differentiation and the role of LIM-homeodomain proteins like Islet-1 in retinal development means I approach unusual or ambiguous cases with a research scientist's instinct to look deeper rather than settle for the first plausible explanation. This has been particularly valuable for patients with inherited retinal disease, uveitic conditions like Behçet's disease and MOGAD, and cases where systemic disease (liver disease, autoimmune conditions) is affecting the eye in ways that are easy to miss. I manage the complete range of vitreoretinal disease, from in-office intravitreal injections for wet macular degeneration and diabetic macular edema to complex vitrectomy surgery for retinal detachment, macular hole, epiretinal membrane, and vitreous hemorrhage. Patients don't need to be referred elsewhere as their disease progresses from medical to surgical management. A practice built for access, not just excellence. I founded Elshatory Retina Associates in 2020 because I wanted a practice where patients could get a retina specialist's evaluation without the long waits and fragmented communication that can be encountered elsewhere. Urgent symptoms, new flashes and floaters, sudden vision loss, are triaged quickly because in retinal disease, time is often the variable that determines the outcome. Peer-recognized standards. I've authored more than 20 peer-reviewed publications and serve as a case reviewer for the Texas Medical Board, work that requires evaluating the standard of care in complex ophthalmic cases. That role keeps me current on what rigorous, defensible clinical decision-making looks like across the field, not just in my own practice. What Patients Can Expect A visit with me typically starts with listening. Vision changes are often described imprecisely, 'blurry,' 'spots,' 'shadows,' and part of my job is translating that description into a precise diagnostic picture using imaging, examination, and a careful history. I explain findings in plain language, show patients their own OCT scans and retinal images so they can see what I'm seeing, and lay out treatment options with honest expectations about what each one can and can't achieve. For surgical cases, I walk through the procedure, recovery timeline, and realistic visual outcomes before the day of surgery, not after. I don't believe in over-treating or under-explaining. If a finding can be monitored safely, I'll say so. If it needs immediate intervention, I'll say that clearly too, and explain why the timeline matters.
