Care delays are operationally frustrating. They are also a direct clinical and financial liability, as recent research in the Journal of General Internal Medicine points out. When patients wait, their conditions worsen, or they seek care elsewhere.

Without standardized triage, physicians spend clinical hours playing administrative catch-up, which eats into appointment capacity and accelerates clinician burnout.

It’s about operating with maximal efficiency to move the needle incrementally,” says Abigail Khan, MD, Ambulatory Access Medical Director at OHSU. 
  3. Routing: Matching clinical urgency with the right care setting prevents patients from bouncing between clinics. For example, Brigham and Women’s uses non-physician teams with standardized protocols for routine conditions like UTIs and back pain to quickly address patient concerns without utilizing doctor appointment slots. OHSU established direct EMR workflows to fast-track post-discharge follow-ups. 
  4. Sustainability: Faster appointments require clear guardrails — such as daily request cutoffs, standardized intake language, and record-tracking support — so speed does not create operational chaos.
  5. Feedback: At Stony Brook Medicine, Rossol uses real-time surveys, virtual focus groups, and waiting-room rounding to capture the qualitative friction that operational dashboards miss. Improved access should be measured not only by whether the system got faster, but by whether patients and staff experienced it as easier, clearer, and more reliable.
