You will receive a phone call as a reminder to fill out the enrollment information on the patient portal. Failure to complete the application process within 12 hours of receiving the enrollment information will result in patient's cancellation of appointment.

Note that I am in network with the following insurance plans: Medicare, Aetna, Cigna, UHC, Highmark/ Magellan Blue Cross, Independence BC, UPMC, Geisinger, Keystone East, Allwell/ Ambetter.
***Out of network charges:
Initial evaluation- $350 (1 hr evaluation)
Follow up visit- $175 (30 minutes)
I can provide a receipt for insurance and reimbursement purposes.
