Can't recommend this doctor / office. After getting an MRI for an issue (approved and paid for by my insurance), the office has completely dropped the ball on getting set up with an orthotic. It has been over two months - they have never followed up, but have let me know they are "working hard on it". Don't go to this office if you want prompt treatment.
