Dr. Cook was born and raised in Johannesburg, South Africa. He received his degree in microbiology and biochemistry at the University of Victoria in Victoria, British Columbia. He received his Doctorate of Optometry at Pacific University College of Optometry where he graduated with distinction and was elected to Beta Sigma Kappa honor society. Dr. Cook joined the staff of EyeHealth Northwest in 1995 and is an adjunct professor at Pacific University College of Optometry.

I've been seeing Dr. Cook for many years and he's always been the consummate professional. He's thoroughly explained options available in glasses/contacts/procedures available for my particular set of circumstances. I also appreciate the friendliness & genuine caring he's always shown. It's a pleasure to visit a doctor with which I can feel comfortable enough to chuckle about life and this wonderful aging process. He's guided me along the path with gentle hands and a good heart, while paying close attention to changes & options for future technological interventions.
In my opinion, he's among the best doctors I've had the pleasure of visiting.
I'd never want to lose him as my physician.

My wait was pretty brief, just enough time to fill out some short paperwork. Dr. Cook was very nice and professional as was the staff that worked with him. The office had a nice environment, it had on site parking and there seemed to be a physical separation between the frame and lens shop and the patient area. I wanted to explore getting glasses online so I was happy that I got the health of my eyes checked and an updated prescription without feeling pressured to buy anything on the spot. The patient rooms were clean and comfortable and I was asked if anything needed adjustment so the staff was looking out for my comfort.
I appreciated being treated primarily as a "patient" and not as a "customer." Dr. Cook was very descriptive with his evaluation with me and I felt I was able to ask questions and that he gave me thoughtful honest answers. He seemed to be very well educated as he was able to describe to me in detail the condition of my eyes as where in the past other doctors would use very general terms and descriptions which left some question marks in my head. I really liked his bedside manner, he spoke clearly and did not make me feel anxious. As a self identified anxious person I would recommend him as a gentle, kind, knowledgable and proficient doctor. I felt really good about my appointment and exam and I plan to see him again for my next check up. I would highly recommend him.

I showed up early to my appointment by 40 minutes. I checked in and the clerk asked if I was going to wait in the office. Within two minutes I was in an exam room, and I was through my appointment and out 10 minutes before my appointment was scheduled to start! Awesome service. Great staff. Visit was quick and painless.

Everyone was great! The office staff was courteous and helpful. I got called back EARLY for my appointment since I was there! There was little if any lag time between the eye tech and Dr. Cook entering the exam room. Everything ran like a well oiled machine. Dr. Cook was awesome! I was very pleased with my experience. Thank you!

Dr. Cook is a very pleasant and nicely mannered young man -- quick and deft with the procedure, and quite good at instructions. I have no fault whatsoever to find with him, or his staff -- I've forgotten her name, but she was also pleasant and good at her job. I would recommend him to anyone who wants a good optometrist in NW Portland.

Dr. Cook and the nurse were both excellent, but the admin staff was mediocre. My wait was actually quite short, but the staff could not seem to find my insurance information, hence the 2 stars for wait time. I have a great vision plan, but had to leave without being able to fill my prescription for new lenses. Worth it to see Dr. Cook though!
