Who are orthodontists?

An orthodontist is a dental specialist with expertise in realigning jaws and shifting teeth. Orthodontists receive intense training in managing dentofacial (including tissues and bones of the jaws and face) development and tooth movement.

Orthodontists identify, treat, and prevent any dental and facial anomalies. They manage issues like misaligned jaws, crowded teeth, teeth that are too far apart or too close together, teeth that meet irregularly or not at all, and teeth that stick out.

Orthodontists treat current issues, can spot emerging issues, and can move quickly to address the problems before they become more serious. Orthodontists often treat patients of all ages, including kids, teens, and adults.

Dental school graduates work as orthodontists. College, dentistry, and orthodontic school are the first three stages of an orthodontist's education. After graduating from dentistry school, orthodontists complete a two to three-year orthodontic residency program. Upon completing all these requirements, orthodontists can opt to become board-certified.

There are more applicants than openings for orthodontic residency. For each opening, there are around 15 applicants. Although dental school offers some orthodontics education, orthodontists train more later.

Are dentists and orthodontists the same?

Both dentists and orthodontists are oral healthcare professionals who treat conditions of the teeth, gums, and jaws. However, the difference in their profession lies in the scope of practice, services provided, and education.

General or family dentists, popularly known as "dentists," focus on your oral health. Dentists treat decayed teeth and place tooth fillings. They also provide services like placing crowns, veneers, and other cosmetic dental appliances that improve the appearance of your smile. While some dentists can offer braces and aligners, they may not necessarily be orthodontists.

Both dentists and orthodontists complete dental school. However, orthodontists are dental specialists who study full-time in an accredited program in their specialty area for two or more years. After completing their specialty training, they are recognized as orthodontists. The training allows them to provide customized orthodontic treatment. Upon completing their specialty training, orthodontists are recognized as American Association of Orthodontists (AAO) members.

Regarding the scope of their practice, general or family dentists offer a broad range of care. In contrast, orthodontists are typically limited to specialty care, i.e., focusing on the correct movement of the jawbones, teeth, facial bones, and soft tissues.

When to see an orthodontist?

Consult an orthodontist near you if you have concerns about your teeth's position or the quality of your bite. They have the specialized knowledge and training required to resolve the issue.

Some issues can be less noticeable. Issues like mouth breathing, teeth clenching or grinding, and difficulty in lip or mouth movements indicate the need for orthodontic treatment. Additionally, if you notice that your jaws move noisily or regularly shift positions, or if you accidentally bite your cheek or the roof of your mouth, you may have an orthodontic problem.

Teeth that meet improperly can result in a face imbalance, where some facial features are out of proportion to others. Orthodontic treatment is frequently able to solve this issue. Early screening doesn't necessitate immediate treatment. Most children start active orthodontic treatment when they are 9 to 14.

What are the benefits of orthodontic treatment?

Orthodontic treatment has various cosmetic benefits. However, orthodontic appliances provide functional and health-related advantages as well. Some of these include the following:

  • Properly aligned teeth are much easier to clean, leading to fewer cavities in the mouth.
  • Braces, aligners, and retainers can help straighten teeth. This can improve chewing and speech function.
  • Temporomandibular joint (TMJ) disorders lead to pain in the joint that connects the jawbones with the skull and muscles that control your jaw movement. Specific orthodontic devices can ease jaw pain caused by TMJ disorders.
  • Properly aligned teeth lead to an even bite, which reduces the risk of conditions like gum disease.

What are the disadvantages of getting orthodontic treatment?

Orthodontic treatment does not have many risks or disadvantages. However, achieving results with an orthodontic appliance may be gradual. Depending on the severity of your condition, when it was diagnosed, and your unique needs and goals, orthodontic treatment may take a few months or even 1-2 years to show results. For some, it may take more than 2 years.

Additionally, it is necessary to follow your orthodontist's instructions on how to clean and maintain hygiene with orthodontic appliances like braces and aligners, as improper cleaning may increase the chances of getting cavities.

What are braces, and how do they help?

By applying consistent pressure on the teeth, braces gradually reposition them into a straighter position to fix how your teeth line up. After wearing braces, you may use a retainer to keep your teeth in a straight posture because it also exerts pressure on your teeth.

Some children might need to wear additional equipment, such as headgear. A horseshoe-shaped wire is used in headgear and fastens to the back teeth. It pressures the back teeth forward, making way for the front teeth. The headgear is typically worn at night as it is essential to keep it on for 12-14 hours daily.

When you wear braces or other orthodontic appliances, you should occasionally expect to feel a little uncomfortable. If it aches, you can take a painkiller. Additionally, the orthodontist gives you wax to conceal any sharp edges on the braces that irritate you or rub against your gums or the inner part of the mouth.

Can adults get braces?

Teeth move in response to forces being placed on them over a period of time. This physiological process does not change, no matter your age. Age does not keep teeth from moving. While orthodontic treatment usually begins during childhood, it can also start during your adult years. Regardless of age, an orthodontist can consider you for orthodontic treatment like braces, retainers, and aligners. Many orthodontic issues can be corrected as easily for adults as children or teenagers.

The critical difference between orthodontic treatment for adults and kids is that children's teeth are still developing and growing, while adults' teeth are fully developed. Given that adults have denser bone tissue than children, the desired results of braces or aligners may take longer. Orthodontic treatment in adults can be as successful as in young children. For adults, orthodontists consider many variables while offering treatment. They consider factors like gum disease or other dental problems, co-existing health conditions, misshapen or missing teeth, and how many fillings a patient has had. The treatment outcome may also be affected by certain medications, habits like smoking, clenching or grinding teeth, and tongue thrust. Before offering suitable treatment, your orthodontist may also coordinate your care with your family dentist and other oral specialists, such as endodontists, periodontists, and oral surgeons.

Suppose you are particularly worried about your aesthetic appearance or how an orthodontic appliance looks on you; worry not! There are many orthodontic options that adults can choose based on their preference or lifestyle, such as clear aligners, ceramic braces (which blend into the color of your teeth), self-ligating braces, or lingual braces (which are typically mounted on the back of the teeth, making them almost entirely invisible but just as effective).

When it comes to seeing the results of your orthodontic treatment, it depends on the complexity of your issues. It may take many years in complex cases, but it can also take as little as eight to twelve months. You can expect to visit the orthodontist every six to eight weeks to monitor your progress and check your overall dental health.

How often must one visit the orthodontist once braces are in place?

Once your braces are in place, you must visit an orthodontist every few weeks. The orthodontist will ensure that your braces are in place during regular visits. However, at subsequent appointments, they may change the wires on the braces to shift the teeth into place if required. Rubber bands stretched between two teeth and worn as instructed by the orthodontist can help to straighten your teeth.

Common orthodontic appliances

Orthodontists make use of a variety of oral appliances to help with orthodontic treatments. Some of these include:

  • Aligners: They are a popular, custom-made alternative to braces, preferred by adults to straighten teeth. Moreover, multiple sets are provided to the patients and are switched within one to two weeks after usage. Each set is a little tighter than the previous ones. They are virtually unnoticeable and removable for eating, brushing, or flossing. Additionally, aligners do not involve brackets or metal wires.
  • Headgear: Headgear is used to restrict the growth of the upper jaw, pull the front teeth back, and hold the back teeth in place. It is commonly used to treat the front teeth while keeping the back teeth in position. Most patients only need to wear headgear in the evening or at night.
  • Lip and cheek bumpers are specialized tools to relieve the pressure of cheeks and lips on the teeth. It prevents them and keeps them away from the teeth.
  • Retainers: They are provided to patients after their orthodontic treatment of braces or clear aligners ends. They are custom-made oral appliances that prevent teeth from drifting out of their positions. They maintain the alignment of straightened teeth while the adjacent gum and bone adapt to the new positions. Moreover, they can be either removable or fixed.
  • Jaw repositioning appliances or splints are placed in the lower or upper jaw to help it close properly. Orthodontists commonly use them to treat temporomandibular joint disorder (TMJ), which causes dysfunction and pain in the muscles involved with jaw movements.

Who should avoid orthodontic treatment?

Individuals with severe dental problems like excessive tooth decay or gum disease are advised not to opt for orthodontic treatment. Placing braces on decayed teeth can further harm your oral health. If you have severe gum disease or other oral health issues, consult a dentist before going for orthodontic treatment. Your dentist will likely recommend orthodontic treatment once all your other oral problems have been treated.

How to prepare for an orthodontist visit?

During your first visit, expect a routine exam. Your dentist may have detected misplaced teeth and advised seeing an orthodontist, for example, if your rear teeth don't fit together correctly. You may go through the following at your initial orthodontic consultation:

These examinations will help your orthodontist choose the best treatment course and orthodontic interventions for you.

Oral health statistics in Detroit, Michigan

When examining particular population groupings across Michigan, there are alarming indications. According to a report published by the Mackinac Center For Public Policy, in 2010, about 42% of third-graders in the metro Detroit area and nearly 30% of third-graders nationwide had untreated dental disease. In metro Detroit, 17% of these third-graders needed immediate dental care. These findings imply that many kids in Michigan need access to dental care due to tooth issues.

However, there is evidence that older adults also require better access to affordable dental care. In Michigan, a 2010 survey found that 30% of seniors had untreated tooth decay, most of which could be corrected with simple dental procedures in a dental clinic. Furthermore, 45% of seniors had gingivitis-related fair or poor gum health. A significant or urgent need for immediate care existed in 18% of senior citizens. According to a 2013 sample of seniors, 17% had severe dry mouth, 16% had severe gingival irritation, 17% had untreated dental decay, and 13% had root fragmentation.

These statistics show that some areas of Michigan may need dental care improvement. Cost is another major obstacle to receiving care. According to nearly 90% of facility administrators of retirement homes and senior centers, financial worries are the main barrier preventing more elders from receiving dental care. Michigan's average out-of-pocket dental price in 2010 was $827, while only 56% of residents have dental insurance. That is 24% more than the $666 national average. Increasing access to necessary care for more people would be possible by lowering dental prices.
