After obtaining a Bachelor's degree from Auburn University in 1992, Dr. Jacobazzi graduated from Life University of Chiropractic in 1999. He then completed a Master's degree in physical therapy at California State University Fresno in 2010, with an emphasis on sports medicine. The only practitioner in the state of Illinois to hold degrees in both physical therapy and chiropractic, Dr. Jacobazzi often gets improved results because of his dual perspective, and his engineering background has lead to his expertise in the field of biomechanics and posture correction. Dr. Jacobazzi maintains his expertise in both fields through continuing education and is an active member of professional societies. I still don't know how they work with insurance, even though it shows as "in - network" and I asked them in the beginning too if they were were in network and they said yes. Shortly speaking, after 2 appointments (which really can be done in 1) I found out that my plan for PT was not fully covered and have to pay a very large amount (almost 2k) out of pocket directly to them... Supposedly I have only 12 sessions through my insurance , which is not true since my insurance said I have unlimited sessions... They are very pushy on the sales side, asking right away if you want to pay the amount upfront or divided in installments, and scheduling ALL your future sessions right away.
Dr. Dan was great and persistent in helping to heal me. I went in with knee pain from running, and he came up with a treatment plan within days. I used to not be able to run a mile without pain and after a few months of treatment, and much better alignment, I am able to run pain free. Would highly recommend his services!

I've seen a few chiropractors over the years in 3 cities and no one has been able to treat my underlying issues like Dan has. Since he's both a PT and Chiropractor, he's uniquely able to diagnose and treat in a very holistic way which is extremely hard to find anywhere else. For example, I've had chronic back issues for at least a decade, with more recent issues splintering off, and can now workout without restrictions and have even started a martial art. Additionally, Dan provides flexibility / discounts to ensure you can handle whatever payment amount you might be responsible for.

I went into HPI with runners knee pain in my right knee. I was typically having symptoms after 5 miles during a 10 mile run. After which i would experience residual pain that would daunt my day for at least three days after a run. HPI was able to take x-rays and upon review identify there was a limb length discrepancy. HPI provide me with a holistic plan to strengthening my core, stretch my legs, lower-body, and treat the area in pain. The plan developed provided me with a reactive and proactive approach to the issue at hand. Since going through the two month physical therapy plan the results have proven to be fantastic and the pain i was experiencing is no longer an issue. The staff has been very personable and accommodating. I greatly appreciate their expertise as it provided me with a pain free running experience.

I came to HPI for a knee issue having never had one before and Dr. Dan identified the problem immediately after a few tests. He laid out a plan and told me exactly how to fix what needed fixing. From the beginning, Dan, Marty, Lauren, Natalie and the team have been brilliant and fun to be with while I rehab my knee. My pain has nearly disappeared and I'm feeling so much better after a few weeks at the clinic. Everyone is always attentive, informative, and motivating. It never feels like a drag going to therapy and it's due to the atmosphere, which feels more like a relaxed gym than a clinic. I prefer this setting because it allows me to do what I need to do with complete ease rather than feel like I'm at the doctors office. The commute for me is rather long and unfavorable but I refuse to switch to another clinic, even though I have options just a few blocks away from my office. This speaks to the quality of attention and resources at HPI. If you're looking for a team that will help you with your needs with top-class therapy and medial intelligence all while making your time enjoyable, then visit them now. These guys are the absolute best. Go get fixed and have a good time.

Before I started to see Dr. Jacobazzi I was going to a commercial physicial therapist and recieved 30 sessions following my knee surgery. Although I could walk, my knee pain continued, so I sought out a new physical therapist. After eight visits with Dr. Jacobazzi, my knee pain is now under control and my recovery has accelerated. In addition, Dr. Jacobazzi got to know me on a personal level and prescribed fish oil after I told him I hate taking pills, not many doctors do that. He also suggested a diet that has helped me lose weight, which has taken pressure off my knee. Dr. Jacobazzi is a physician who practices continued learning, so when you come to HPI Chicago you know you are receiving current medical techniques, advanced recovery equipment, and consultantion. I recommend anyone who is trying to better themselves and in need of physical rehabilitation to come to HPI Chicago.
