If you plan on using Cigna, United Healthcare, Optum or Oscar insurance to pay for your session, then please provide me with your date of birth and a photo or pdf of the front of your insurance card. After I verify your co-pay and deductible, I will email you back to confirm your fee.
If you are not insured by Cigna, United Healthcare, Optum or Oscar, then you will pay the out-of-network fee and then submit a monthly superbill to your health insurance company for reimbursement. To reiterate, I am NOT in-network for Blue Shield, Anthem Blue Cross, Healthnet, Kaiser, Medical, Medicare, EAP or any other health insurance company aside from Cigna, United Healthcare, Optum or Oscar.*
In an effort to be fair, I offer a sliding scale based on your or your family’s gross income as stated on the first page of the previous year’s tax return. I charge .1% of your annual income per session with a floor of $250 and a ceiling of $400 per session (not per hour).
