My first impression unfortunately was the receptionist who was severely ill with the Flu… I was extremely taken back that she was at work - with a mask, told me how sick she was, and said she couldn’t afford to take off. I stayed as far away as possible, but STILL. putting patients at risk like that was irresponsible and inconsiderate to the max. The nurse who prepared me for the appointment was an absolute doll, very professional and kind. The doctor was very straight to the point and knowledgeable. I would go back, hopefully with a receptionist who isn’t extremely sick next time…

Dr. Adam Chubak attended Yeshiva University for undergraduate, where he graduated with honors and deans list for all four years. After undergrad he spent a year doing lab research at NYU under the direction of Dr. Daniel Rifkin analyzing TGF-Beta and its effect on heart development. Dr. Chubak then attended VCU for medical school where he led many student interest groups. After medical school, Dr. Chubak started his residency in Vascular Surgery at Mt. Sinai, completing his intern year in General Surgery and another year of vascular surgery.
Ultimately deciding to become an ophthalmologist, Dr. Chubak went on to do a pre-residency fellowship in Ophthalmology at SUNY Downstate, and then joined SUNY downstate for ophthalmology residency. In his final year he was a chief resident and was the primary surgeon for many cataract and anterior segment procedures as well as emergent trauma. Dr. Chubak completed his corneal fellowship under the guide of Dr. Theodore Perl, focusing on transplants, refractive cornea surgery, and anterior segment surgery. He is involved in research and is a member of the AAO, ASCRS, and other ophthalmological professional organizations.

The office was clean and the administrative staff were great, everyone was very friendly and helpful. There wasn’t many people so I actually was seen about 10 minutes early. The doctor I saw was professional but not polite or kind by any means. He did his job efficiently, but never explained what he was doing which I found off putting. Overall, I wouldn’t make an appointment with this man again, but the rest of the office was great.

I am very happy I saw Dr. Chubak. He is a very smart doctor and was informative about the genetic eye condition I have. I was able to easily schedule follow-up testing for future months. I am recommending my sister to his practice for baseline testing as well.
