The Wellness Connection brings convenient, patient-tailored healthcare to you, whether it is via Telehealth or in the comfort of your home or office. We provide service for patients in Arizona only due to licensing. No controlled substances offered during telehealth appointment on first visit. There is a 24 hour cancellation period, or you will be charged for a missed appointment.
Using guideline driven standards we want to help you reach your health goals. Not all concerns can be managed via telehealth; some do require in person visits, which can be provided in the Greater Phoenix area, or at times with a visit to the urgent care or emergency room. Controlled substances are rarely prescribed via telehealth. The Wellness Connection only offers services for those currently in the state of Arizona.
We are passionate about healthcare. Every person should have access to good quality healthcare delivered in a respectful, caring, and compassionate manner where you have a say in the care you receive. We bring over 35 combined years of experience to you. Services include treatment of illness and injury as well as annual physicals and maintenance of chronic conditions such as high blood pressure, diabetes, and elevated cholesterol to name a few.
Jann has been practicing one form of medicine or another since 1989. She started as a paramedic with the fire department and AirEvac before transitioning into the role of an emergency room nurse. Her quest for knowledge guided her through graduate school at Grand Canyon University where she graduated with honors as a Family Nurse Practitioner. Jann loves staying active through hiking, exploring Arizona, golfing, and playing lacrosse with her son. As a mother of four, she understands the challenges of both caring for ill family members as well as that of staying healthy as a parent.
