Everyone at that office is amazing. Clean quiet office. I was treated kindly and with respect. Samantha gave me the time and space to let her know my concerns. She listened and offered amazing advice. I left feeling like she has my back and best interest in mind. Glad I found this place!

I am SO happy to have had a great experience and finally have a PCP! Samantha heard all of my concerns and made recommendations/referrals so I can have well rounded care for all of my needs. Aside from urgent care when needed, I haven’t had routine care since well before the pandemic. I’m looking forward to continuing my health journey with Samantha and the staff here!

I was not sure what to expect and was anxious about the appointment. I had nothing to worry about. Quevedo was a great listener, did not rush me and was able to answer my questions or concerns. Great bedside manners and smooth visit. I also made a mistake of booking in person but the office answered right away when I called and scheduled me for telehealth visit as I wished.

I appreciate being able to schedule an appointment for after work hours! I was seen right away by the medical assistant/PT who gave really helpful exercises. Sam has excellent bedside care. I’ve never felt so comfortable with a medical provider. I’ve been searching for a PCP who can explain medical terminology in plain language, actively listen, and address any issues/questions. Glad I found Sam and 5th Ave Health!

I loveeeddddd Sam ! I didn’t know what to expect being that this was my first time at her office and this type of office in general . It was such a quick process . I loved the receptionist she was respectful and attentive , the medical assistant was lovely made me comfortable and Sam was amazing I loved the fact that she seemed near my age she was relatable listened to me was ready to help how ever she could and gave me great vibes I was in and out in no excessive wait
