Although my Psychiatric training in Psychiatry ages ago (1984-1990) at Ohio State University Hospitals has firmly grounded my private practice in mainstream science-informed medicine, over the years I have come to appreciate that caring for my patients has to get beyond diagnostic labeling and penetrate deeply into the "heart-mind" of the authentic human being. A collaborative partnership of shared responsibility seems to facilitate the process of exploration, understanding and ultimate reconciliation with my patients' unique lives and sense of who they truly are. We are "fellow travelers" in the flow of life, our paths crossing here and there as we share experiences and insight, this companionship informed and supported by the teachings of Pure Land Buddhism. Ordination as a Jodo Shinshu Buddhist priest was the affirmation that I, too, am a vulnerable human being who receives unconditional Universal Compassion just like everyone else. This shared understanding of how kindness works in our lives is the essence of healing, the birthright of every suffering human being.

What should a new client know about working with you?

After a fairly lengthy but simple paperwork is completed (either online or on paper), the first interview is scheduled for 90 minutes to gather clinical information, formulate a diagnostic framework and an initial comprehensive care plan. This may include, but not limited to: acute stabilization of the most disruptive symptoms, pharmacogenetic testing, preliminary trials of psychotropic medications, psychoeducation, behavioral assignments, relevant medical tests and/or referrals, and psychotherapeutic goals and approaches. I welcome pretty much anyone for as long as they are willing to work with me; I do tend to go right to the "heart of the wound;" I'm pretty bad at keeping up appearances (unless I'm in my Halloween costume!) I come clean with my patients about my limitations as a clinician, such as when they require a more intensive level of care or need to see another professional. I expect my patients to be honest with me and express their opinions freely about their concerns, questions, frustrations, and criticisms and appreciation. My established patients call me by my first name and have direct access to me 24 hours/day, 7 days a week. My services include: diagnostic interviews, psychopharmacology, individual/couples/family therapy, mindfulness-based approaches, collaboration with other professionals, including schools and social services, pharmacogenetic testing, and Transcranial Magnetic Stimulation for Treatment-Resistant Depression.

What is your typical process for working with clients?

The initial psychiatric interview includes history-taking, past psychiatric history, substance use, medical history, family history and genogram, mental status examination, and clinical formulation on which an initial treatment plan is constructed. For children younger than 8, I usually conduct 2 interviews: first with the parent(s) alone, the second with the child (with and without the parent(s). Additional developmental history is obtained for patients under the age of 18 (and in some young adults). Our treatment goal in the first meeting is to stabilize the most life-disrupting target symptoms and prevent further crises in the near-future. When things are more stable, we explore the most salient contributing and predisposing factors that require behavior change while identifying the patient's core beliefs that play a critical role in their healing process. The ultimate goal is for the patient to reach a more livable and practicable paradigm that promotes their well-being and self-reconciliation.
