Development of a personalized treatment plan, which may include medication management, therapy referrals, lifestyle recommendations, or a combination of these. Education and Decision-Making: Clear explanation of your diagnosis and treatment options, including benefits, risks, and alternatives. Empowering you to make informed decisions about your care. Implementation and Monitoring: Starting the agreed-upon treatment plan and closely monitoring your progress. Regular follow-up appointments to assess effectiveness, side effects, and overall well-being. Adjustment and Continuity of Care: Adjusting the treatment plan as needed to ensure the best outcomes.
