About Ellen Lasser
If you are a new patient, please note that the office requires immunization records and past pediatric records 14 days PRIOR to the date of your visit. These may be brought into the office in person or faxed (please call the office number provided in your confirmation email to facilitate this). Also, kindly contact your carrier to change your child's primary care provider to our office.
