Patients will be notified within 24 office hours of booking for intake paperwork and to verify insurance information.
Controlled substances (stimulants, benzodiazepines, opioids) will NOT be prescribed outside of a 60 mile radius. Call the clinic for clarification before booking to avoid fees.

For pediatric patients we do sport physicals, school physicals, and sick visits.
Erica Evans is a dedicated Family Nurse practitioner with over two decades of experience in emergency room medicine, primary care, urgent care, and behavioral Health.
A Cincinnati native, Erica earned both her undergraduate and graduate degrees at the University of Cincinnati, graduating magna cu laude and making the dean's list several times. She recently completed the University of Cincinnati's Psychiatric Mental Health Nurse Practitioner post master's certificate program, with board certification pending. Her career includes notable roles in Cincinnati, Ohio at Cincinnati Children's Hospital, in Tampa, Florida at TGH Fast Track Urgent Care, Walmart Health, and psychiatric hospitals in both Cincinnati and Tampa.
Erica is passionate about patient care, valuing shared decision-making and self-driven treatment plans to help patients achieve their health goals.
In her free time, Erica enjoys fishing and traveling with her husband and their children.

I am so upset by this provider!!! We had a great session and was so happy to finally find a mental health provider who listened & formulated an individualized POC. However, she carelessly prescribed me the wrong formulation of medication, forgot to order my blood work she said she would order. At first I was super understanding, she’s human, we all make mistakes. BUT I have been trying to contact her since and no response. Left multiple messages via E portal & phone. Now I’m stuck with the wrong medication & in the same boat I was prior to booking :(

I LOVE this clinic and everyone I have had contact with here. My provider Evans treats me with respect and looks for my input before making final decisions about medications or referrals. My visit was extremely helpful and efficient. I went in for a couple reasons. I had issues with my stomach and acid reflux, and we got straight to the bottom of what beds to be done and I’ve been ok ever since. My second issue was my back pain, sciatic nerve issues. Would highly recommend
