Dr. Michael Dionne, PMHNP, DNP

Adult Psychiatric & Mental Health Nurse Practitioner
NH
I'm Michael Dionne, RN, DNP, PMHNP-BC — founder of Reforge Psychiatry. I provide telehealth psychiatric care built for veterans, active-duty service members, and first responders dealing with PTSD, trauma, anxiety, depression, and sleep issues. I served three years as an Army infantryman and three more as an Army Nurse. Left in 1997, thinking PTSD was permanent. Later, in critical care at Travis AFB, I saw veterans return from deployments with 100% ratings and just a "manage it" plan. Then I got my own PTSD diagnosis from training — not combat. I know many veterans think their experience "doesn't count." It does. The brain doesn't check the source. Science has moved forward. I focus on evidence-based psychiatric care: precise medication management when it fits, plus approaches that target real progress — like understanding nervous system responses and processing trauma effectively. For some, we start with or add targeted supplements backed by evidence. Everything is tracked, reasoned, and built around you — no pill mill, no random guesses. Reforge serves those told to "just cope." I know VA systems, disability ratings, and documentation. We track progress and ongoing care without risking benefits. No judgment. No runaround. Just care that returns control. Precision. Progress. Control.

What should a new client know about working with you?

Expect direct, no-nonsense telehealth psychiatric care built for veterans, active-duty, and first responders. I speak peer-to-peer — no soft clinical talk, no pity. You've likely dealt with systems that failed you; I get the skepticism and respect it. My approach centers on evidence-based treatment that targets real progress, not endless management. We focus on precise medication management when it supports your goals. I incorporate understanding of nervous system responses (polyvagal-informed) and effective trauma processing (like IFS) without requiring you to relive everything in detail. For some, we explore ketamine-assisted options or evidence-backed supplements as adjuncts — but only if clinically appropriate and tracked—no pill mill, no random guesses. Appointments are straightforward: we discuss symptoms, history, and what you've tried and build a plan together. Progress is measured and adjusted. I know VA systems and disability ratings — we document carefully without risking benefits. If you're ready for effective care that returns control, this fits. If not, I'll say so. Precision. Progress. Control.

What is your typical process for working with clients?

Everything starts with a free 15-minute telehealth consult (video or phone) — no commitment. We talk about what you're dealing with, symptoms, past treatments, and whether Reforge is the right fit. I'll be direct: if it's not, I'll point you elsewhere. If we move forward: 1. Brief intake: You share medical history, current meds, and what you've tried. 2. First full appointment (45-60 min usually): We review in detail, clarify diagnosis if needed, and build your personalized treatment plan together. This includes evidence-based options like precise medication management (core for many), strategies for nervous system regulation, trauma processing approaches, or adjunct supports when they fit. 3. Follow-ups: Most are 20-30 min telehealth visits focused on med management, progress tracking, and adjustments. Early or complex visits run longer. No heavy weekly talk therapy — emphasis on practical, outcome-focused care. Sessions are collaborative: you drive what matters. I explain reasoning plainly, track changes, and adjust based on real data. Telehealth means no drives, no waiting rooms — just effective work from where you are.
