I earned my Bachelor of Science in Nursing and Juris Doctor degrees from the University of Santo Tomas, and my Master of Science in Nursing—Psychiatric and Mental Health Nursing—from Walden University. Since 2004, I have worked extensively in various inpatient acute care environments, including Medical and Behavioral Psychiatric Inpatient Units, serving Adolescents, Acute Adults, Geriatric Med-Psych, and Psychiatric Intensive Care populations, with a focus on significantly impacted, underserved groups. Over the years, I have cultivated a refined skill set and a profound philosophy centered on providing compassionate and empathetic care to a diverse range of patients grappling with psychiatric and dual diagnoses, as well as behavioral health challenges. My passion lies in helping individuals identify the incapacitating beliefs holding them back and guiding them to move past these barriers, fostering a greater sense of power, mastery, and triumph. What to Expect Working with Me As a licensed mental health provider specializing in mood disorders and neuromodulation treatments, my focus is helping patients find effective care when traditional treatments have not provided enough relief. A central part of my practice is Transcranial Magnetic Stimulation (TMS), an FDA-cleared treatment for depression and obsessive-compulsive disorder, combined with comprehensive psychiatric care. 1. Personalized Care Every patient’s situation is unique. I take time to understand your symptoms, treatment history, and goals so we can create a clear and practical treatment plan together. 2. Advanced TMS Therapy TMS is a non-invasive brain stimulation treatment that targets the brain circuits involved in mood regulation. Magnetic pulses stimulate areas that may be underactive in depression, helping restore healthier brain activity without the systemic side effects often associated with medications. 3. Careful Evaluation Before starting TMS therapy, every patient receives a comprehensive psychiatric evaluation. Brain mapping during the first session helps identify the precise location for stimulation. 4. Integrated Mental Health Care While TMS can be highly effective, some patients benefit from a broader approach that may include medication management or therapy to support long-term improvement. 5. ADHD Evaluation Some patients experiencing depression or burnout also struggle with attention and executive function challenges. ADHD evaluation and treatment are available through secure telehealth across California. TMS therapy is provided in person at our Bay Area clinic, with additional psychiatric services available virtually across California. My goal is to help patients restore brain balance, improve mood and focus, and regain the ability to fully engage in their lives.

What is your typical process for working with clients? At Exxceed Wellness, our process is designed to make care clear, structured, and supportive from the very first visit. My practice focuses primarily on evaluating and treating depression and OCD with Transcranial Magnetic Stimulation (TMS), while also assessing related conditions such as anxiety or ADHD when attention or executive function challenges are part of the picture. The first session is a comprehensive psychiatric evaluation. We review your history, current symptoms, previous treatments, and daily functioning. I ask detailed questions about mood, motivation, sleep, stress, and cognitive functioning to understand the full clinical picture. If attention difficulties are present, we may also screen for ADHD to determine whether it may be contributing to your symptoms. Based on this evaluation, we develop a personalized treatment plan. For patients struggling with depression or OCD that has not responded well to medication, we may discuss whether TMS therapy is appropriate. TMS is a non-invasive brain stimulation treatment that targets the brain circuits involved in mood regulation. If TMS is recommended, the first treatment visit includes brain mapping to identify the precise area of the brain for stimulation. Treatment sessions are then scheduled regularly at our Bay Area clinic. Sessions are brief, performed while you are awake, and patients can return to normal activities afterward. In addition to TMS therapy, I provide psychiatric medication management and supportive therapeutic guidance when appropriate. For patients who also have ADHD symptoms, evaluation and treatment can be provided through secure telehealth across California. Throughout the process, my goal is to provide clear guidance, evidence-based treatment, and a structured plan so patients understand their options and feel confident moving forward.

The doctor spoke over me, asked me point blank to "describe the most traumatic thing that has ever happened to me," and proceeded to not provide any care for how distraught it made me. I had to firmly re-state things and interrupt the doctor, as she made many attempts to insert incorrect information into the record. Then, I was given a followup appointment the next day without being told I would need one--to go over my QB test results, a test I had only just been told about and now needed to take, after having a very upsetting session with the doctor. Overall, I have never had a worse experience with a psychiatrist, and I have seen MANY psychiatrists. Steer clear, as well as you can.

I was a little taken aback by Dr. Nef's blunt demeanor during our initial appointment, which felt not harsh but not very welcoming either. I understand the need for individual assessments, but the complete dismissal of the conditions I have already worked hard to have recognized through testing and diagnoses over years made me feel like I was back at square one when I really just needed a refill for my medication. I also provided my gender identity and pronouns beforehand and stated my identity during the appointment but she continued to misgender me in her writing. Overall she was professional and nice enough but I didn't feel like I had the opportunity to provide a comprehensive picture of my mental health/conditions.

Not only was I able to get in for a last minute appointment in the middle of the day on a weekend, but Dr. Nefretiri was extremely open to hearing my past treatments and what I had expressed had worked previously medication wise.
