Rachel Hart, NP, PMHNP-BC

Rachel Hart, NP, PMHNP-BC

Adult Psychiatric & Mental Health Nurse Practitioner
186 Lincoln St, Ste. 112, Boston, MA
I’m a Board Certified Psychiatric-Mental Health Nurse Practitioner with a background in emergency medicine and experience providing care in crisis services, outpatient settings, and telepsychiatry. Before becoming a PMHNP-BC, I worked for years as an Emergency Department nurse in high-acuity settings (including Level 1 and Level 2 trauma centers) and served as a First Lieutenant in the U.S. Army Reserves, where I helped train and lead medical teams. I bring a calm, steady, and practical approach to care, listening carefully and focusing on concrete next steps.

What should a new client know about working with you?

You can expect a style that is direct, compassionate, and collaborative. I take time to understand what’s bringing you in and what’s been happening in your life—stress, sleep, medical history, past treatments, and what you want to be different. I’ll share my clinical thinking in a clear way, welcome your questions, and we’ll make decisions together so you feel informed and supported (not rushed or talked at). Because of my background in emergency and crisis care, I’m especially attentive to safety, stabilization, and practical next steps—whether you’re feeling overwhelmed, stuck, or not sure what’s going on. My goal is for you to leave appointments with a plan that feels manageable and tailored to you.

What is your typical process for working with clients?

We begin with a thorough initial evaluation, where I learn your story, review symptoms and timeline, discuss relevant medical factors and current medications, and complete a risk/safety assessment. We’ll talk through likely diagnoses and the full range of treatment options including therapy supports, lifestyle and sleep strategies, and medication when appropriate—then agree on a plan with clear goals (for example: improved sleep, fewer panic episodes, steadier mood, better day-to-day functioning). Follow-up visits are structured and goal-oriented. We track what’s improving, what’s not, and any side effects or concerns, then adjust thoughtfully. If you need additional support (therapy referral, higher level of care, or coordination with primary care), I’ll help you navigate those steps so you’re not doing it alone.
