What is your typical process for working with clients?
Please note all required intake forms must be filled out and signed through Zocdoc and this practice requires a payment method to be listed on file before your first appointment time. You will receive an email shortly from the practice asking you to fill out a payment method which must be completed before your first appointment time.
For insurance payments: Providing a payment method on file is required for your co-pay.
For private-pay patients: The fee is $200 for initial appointment and $150 for follow-ups.
We reserve this appointment time just for you. Because last-minute cancellations and missed visits make it harder for other patients to access care, we ask that you let us know at least 24 hours in advance or this will result in a $50 no-show cancellation fee.
