The company that provides your vision benefits, your ID number, your group number, the primary member’s full name and date of birth. If your benefits are provided by VSP, we will also need the primary member’s last four digits of his/her SSN.
We need this information for 2 primary reasons:
a) If we cannot verify your vision benefits before your appointment, there is a chance that you may be ineligible for your vision exam.
b) If you are interested in using your vision benefits for optical hardware or contact lenses, it is important for us to determine the amount and your eligibility by contacting your vision benefits company.
2) If you currently wear/are interested in contact lenses, we have an additional contact lens evaluation fee.

. Dr. Sue is approachable and understanding. I love that she explained the different tests and procedures she did on me in realtime. Staff are all very friendly and helpful. Everyone is extremely knowledgeable of insurance coverages and they will work with you to find the best solution to fit your needs.

Dr. Sea really impressed me with her professionalism and knowledge. She addressed my questions and concerns in a very caring manner, and even entertained a few personal questions on why she chose this career path. It was nice to get to know her. Great visit. I woke up in the middle of the night one day with my left eye swollen and blurry vision. Dr. Sea saw me the next day and prescribed me an antibiotic ointment for infection. Felt much better the next day.

Everyone at the office was very nice and accommodating. Was very willing to stay until I was completely done choosing frames. Eye exam was very thorough and everything was explained to me.
