Getting to know Dr. Thomas Finch

I am a board certified psychiatrist and psychotherapist with expertise in the treatment of mood disorders such as depression and bipolar disorder, anxiety and panic disorders, ADHD, self esteem issues, relationship difficulties, work related stress, phase of life challenges, and other psychological and emotional difficulties. My focus is on treating the whole individual, and I utilize both talk therapy and, when necessary, medication, to optimally tailor treatments to individual patient needs. I earned my medical degree from Weill Cornell Medical College, and completed by training in adult psychiatry at New York Presbyterian Hospital, Weill Cornell. I also hold an MSc in Medical Anthropology from the University of Oxford and an MBA from Columbia University. I previously served as attending psychiatrist at both NYU Langone Medical Center and New York Presbyterian Hospital Weill Cornell, where I supervised and taught residents and medical students. Although I do not participate in any insurance plans, I do provide necessary documentation to submit for out of network insurance reimbursement, if this option is available to you. **I will reach out to you via telephone for a brief conversation prior to confirming your appointment.** ***Please note that I am not in-network with any insurance plans. What should a new client know about working with you?

I provide initial and second opinion consultations, as well as ongoing medication management, psychotherapy, and combined medication management and psychotherapy. I primarily offer supportive and insight-oriented psychotherapy, utilizing a psychodynamic framework, to help people better understand both themselves and whatever difficulties they may be having. My approach in this is friendly, patient-centered, and full of curiosity. This kind of therapy is typically done either weekly or twice weekly. My approach to medication management is similarly patient-centered. I work with you to determine if a medication trial is worth considering, while being sensitive to the complex feelings involved in considered whether to begin medication. I also work with patients to reduce or simplify confusing or complex medication regimens. Although I treat many conditions in my practice, I have a few areas of specific interest and expertise within my practice. These include: -Working with individuals with ADHD to both optimize their medication regimen and to explore the unique challenges that people with ADHD can experience, including job and school responsibilities, relationships, day to day tasks, shame and anxiety around making mistakes, etc -Working with people who have difficulty with interpersonal relationships, including friendships and romantic partnerships. This can include difficulties trusting others, difficulties with dependence, and other kinds of interpersonal anxieties. -Working with people who remain cautious around Covid, and who have felt ostracized or had significant ruptures with family or close friends

What is your typical process for working with clients?

An initial consultation takes 90 minutes, typically divided into two separate 45 minute appointments. In part, this helps improve your insurance reimbursement if you are utilizing out of network benefits. During an initial consultation, I will take a detailed history, get to know you as a person, and provide my thoughts and recommendations at the end of the process. From there, we will work to develop a treatment plan that can include psychotherapy, a medication trial, or both. In the case where a specific kind of treatment or therapy that I do not have training in may be indicated, I will facilitate a referral to a colleague who would be better suited. For patients who have established relationships with a therapist, I am happy to provide medication management and collaborate with that therapist as needed.
