Dr. Yasleny Baker, DNP

Adult Psychiatric & Mental Health Nurse Practitioner
121 E Silver Spring Dr, Ste 207, Whitefish Bay, WI
I'm a board-certified Psychiatric Mental Health Nurse Practitioner with a Doctor of Nursing Practice degree and a decade of experience helping adults navigate mental health challenges. I founded Mindara Modern Psychiatry with a simple belief: effective psychiatric care starts with understanding the person, not just treating a diagnosis. I specialize in treating ADHD, anxiety, depression, PTSD, OCD, and other mental health conditions in adults ages 18 and older. My approach is thoughtful, collaborative, and grounded in evidence-based care. I take the time to understand your symptoms, personal experiences, and treatment history before recommending a plan tailored to your needs. I believe medication can be an important part of treatment, but more medication isn't always the answer. Sometimes the most meaningful progress comes from clarifying a diagnosis, addressing contributing factors, or making thoughtful adjustments to an existing treatment plan. My goal is to help you understand your mental health, feel confident in your treatment decisions, and achieve meaningful improvements in your daily life. Whether you're seeking treatment for the first time, struggling with medications that haven't worked, or looking for a provider who will listen and explain your options, I welcome the opportunity to work with you. I offer psychiatric evaluations and medication management in English and Spanish, with in-person appointments in Whitefish Bay and convenient telehealth services throughout Wisconsin.

What should a new client know about working with you?

Welcome to Mindara Modern Psychiatry! I provide psychiatric evaluations and medication management for adults ages 18 and older. Before your first appointment, you will receive instructions to register for the patient portal, complete required intake and consent forms, and submit your insurance information. These steps must be completed before your appointment so that your information and benefits can be reviewed. Please be prepared to discuss your current symptoms, previous psychiatric treatments, and medications you have tried. If available, a list of prior medications and your response to them can be especially helpful. Treatment recommendations, including medication decisions, are based on an individualized clinical assessment. Scheduling an appointment does not guarantee that a specific medication, including a controlled substance, will be prescribed.

What is your typical process for working with clients?

Your initial psychiatric evaluation includes a comprehensive discussion of your symptoms, mental health history, previous treatments, and current concerns. Together, we will explore possible diagnoses and develop an individualized treatment plan. When medication is appropriate, I explain the available options, expected benefits, potential side effects, and how we will evaluate your response. Follow-up appointments focus on monitoring symptoms, assessing medication effectiveness and tolerability, and making adjustments when needed. Supportive psychotherapy may also be incorporated into visits to address current stressors, coping strategies, and behavioral patterns. Follow-up frequency depends on your clinical needs. Appointments are generally more frequent when starting or adjusting medications and may become less frequent once symptoms are stable.
