I am Board Certified in Adult and Geriatric Psychiatry. I specialize in treating depression, anxiety, and mood disorders in adults, as well as helping with issues related to socializing and dating in the 21st century. I provide geriatric diagnosis and care for dementia, depression, anxiety, and all mood disorders associated with aging. The main goal is successful aging and how to attain it. In my practice, I provide patients with individually tailored treatment to improve outcomes, success, and full enjoyment of life. During your initial 30-60 minute evaluation, Dr. Collazo will assess your treatment needs. At that point, she may continue with you as a patient or decide that your needs would be better met by referral to a therapist or another type of healthcare provider.

Subsequent visit fees depend on the length of your time for your visit which will be determined by your condition: $200 15-20 Minutes medication follow up $300 30-45 Minutes session Please be aware that there is a $100 cancellation fee for initial evaluations canceled within 24 hours of the visit and you will be charged the full visit fee for canceling any regular visits within 24 hours of the visit. Please fill out and return the self-pay agreement attached to continue your booking.
