Who is an OB-GYN?

An OB-GYN, also known as an obstetrician-gynecologist, is a medical professional who deals with women's reproductive health. Throughout a woman's life span, from puberty to menopause, her body experiences various physical and mental changes. An OB-GYN is a medical specialist who provides care and support to women through these phases of their lives, including during pregnancy. They are doctors who have extensive training in obstetrics and gynecology. People train in both these fields simultaneously to provide complete, comprehensive care to women.

Obstetrics is a medical field that treats and cares for women before, during, and after pregnancy. It includes prenatal and postnatal care and nurturing for newborns. Gynecology deals with a woman's reproductive system's health and developmental stages.

They offer various specialized medical services for women and help identify, treat, and prevent diseases related to their reproductive system. Most women consider an OB-GYN to be their primary care doctor. They perform different tests and health screenings to identify, treat, and care for a woman's reproductive system.

Educational qualifications of an OB-GYN

An aspiring OB-GYN goes through years of rigorous clinical training and education. They must complete a four-year med school where the first two years involve studying anatomy and physiology. In the final two years of medical school, they must work as apprentices in clinics and hospitals for doctors.

Following medical school graduation, they must complete a residency program accredited by the ACGME (American Council for Graduate Medical Education) for three to four years. During the residency program, they get to choose their career specialty. After their training, they must take an exam conducted by the American Board of Obstetrics and Gynecology to get board-certified. They must meet all the state's medical board requirements to gain state licensure. Additionally, they must earn education credits yearly to keep their license active.

What does an OB-GYN do?

Most OB-GYNs are trained surgeons who perform many critical medical procedures like:

  • Cesarean deliveries
  • Hysterectomy
  • Use of instruments during childbirth
  • Surgeries for pelvic organ injuries
  • Removing ovarian cysts and uterine fibroids

Some of the routine procedures that they perform daily include:

  • Conducting tests for STI, pap smears, etc
  • Fertility treatments like egg-freezing or egg retrievals for IVF
  • Counseling for various conditions
  • Perform pelvic ultrasounds
  • Treat urinary tract infections and incontinence
  • Regular health screenings
  • Mammograms and other breast cancer screening
  • Perform blood tests for common medical conditions
  • Check and treat mental health issues
  • Prescribe medications for common ailments
  • Referring patients to other specialists if required

What conditions do OB-GYNs treat?

OB-GYNs diagnose and treat a wide range of conditions, including the following:

  • Endometriosis: Endometriosis is a common illness in which tissue resembling your endometrium (the lining of your uterus) grows around your abdomen and pelvic area, such as in the fallopian tubes, vagina, intestines, and rectum. When that happens, it can result in uncomfortable symptoms like excruciating menstrual cramps, abdominal or back pain during and/or in between your periods, and painful bowel movements.
  • Ovarian cysts: Ovarian cysts are sacs full of fluids or semi-solid materials that form in one or both of your ovaries. These cysts are of many types, most of which are benign. While smaller ovarian cysts do not cause any symptoms, larger cysts can lead to painful periods, pain during sex, pelvic pain, and a dull backache.
  • Uterine fibroids: The most common non-cancerous tumors, uterine fibroids are muscle and tissue growths in or on the uterine wall. Most small fibroids do not cause any symptoms, whereas larger fibroids can result in bleeding between your periods, excessive menstrual bleeding, constipation, and frequent urination.
  • Pelvic pain: Pelvic pain in women refers to pain in the region of reproductive organs. This condition can happen due to diverse reasons, including appendicitis, urinary tract infections, sexually transmitted infections, and psychogenic pain. Pelvic pain can also indicate pregnancy, ectopic pregnancy, miscarriage, and pelvic inflammatory disease. Sometimes, other symptoms, such as fever, hip pain, vaginal and/or rectal bleeding, and painful urination also accompany this condition.

Common types of OB-GYN specialists

The American College of Obstetrics and Gynecology recognizes seven subspecialties for an OB-GYN. OB-GYNs who opt for critical care or hospice/palliative care for patients with acute or life-limiting conditions. They can also specialize in family planning to help and educate people on sexual health, planned parenthood, and contraception. However, some of the common types are:

  • Maternal-fetal medicine specialists handle high-risk pregnancy complications. They can nurture and care for a baby and its mother.
  • Reproductive endocrinologists are also known as infertility specialists. They treat infertility and hormonal complications through various methods like IVF.
  • Gynecologic oncologists treat cancers of the female's reproductive organs. They manage and treat ovarian cancer, cyst cancer, etc.
  • Female pelvic specialists attend to, treat, and diagnose injuries and disorders of a woman's pelvic floor and its related organs.

When should you see an OB-GYN near you?

One of the most effective ways to stay fit is by practicing preventive care. Women who are healthy and not experiencing any reproductive health issues should have an annual preventative checkup. During this visit, your doctor may conduct evaluations, immunizations, or lab tests for specific diseases. Your age and health will determine which tests you need. Additionally, your doctor will perform a pelvic exam and may request further tests or screenings based on your individual needs.

It would be best to visit your OB-GYN whenever there are changes in your reproductive cycle, including your first menstrual cycle, pregnancy, perimenopause, and menopause, in addition to your annual checkup.

These symptoms may indicate abnormal changes, such as:

  • Changes in menstrual bleeding volume or frequency
  • Unusual cramping
  • Pain during urination
  • Pain during intercourse

Consider the age and reproductive goals when visiting your OB-GYN. If you're sexually active, your OB-GYN can provide birth control, safe sex tips, advice on preventing or treating STIs or HIV, HPV vaccination, treatment and prevention of pain during sex, abnormal vaginal discharge, and urinary tract infections.

In case you are pregnant or planning to be, your OB-GYN can help with prenatal care, infertility treatment, genetic testing, diet advice, labor and birth options, breastfeeding advice, and postpartum depression.

If you experience pain, cramps, or irregular bleeding during menstruation, consult an OB-GYN for advice on managing symptoms, evaluating cysts and fibroids, addressing pelvic support problems, and getting help with mood swings and premenstrual syndrome.

If you have experienced or are experiencing menopause, your OB-GYN may help with low libido, vaginal dryness, hot flashes, bone loss, incontinence, and hormone replacement therapy.

Some women choose to see an OB-GYN instead of a primary care physician. However, not all OB-GYNs are comfortable serving as primary care physicians. It is essential to discuss with your doctor if they are the right fit for your general health needs.

What should one expect during a visit to an OB-GYN?

An OB-GYN visit today focuses on overall health rather than just routine exams. While the doctor may not require pap tests, pelvic exams, and breast exams at every checkup, the consultation covers physical and mental well-being.

A gynecologist reviews personal and family medical history to assess health risks. The OB-GYN may also discuss lifestyle factors like diet, exercise, substance use, housing security, and safety at home. Mental health is a key focus, with questions about work stress, home responsibilities, and childcare. Screening for anxiety and depression is common, and the doctor may provide referrals to specialists if needed.

Patients can discuss symptoms such as unusual discharge, painful urination, or menstrual issues. The doctor may recommend tests like urine analysis or blood work. Treatment options are explored, with follow-ups if necessary.

For those considering pregnancy, discussions include fertility timelines and preconception care. Those seeking contraception can explore various birth control methods based on individual needs.

Consultations address perimenopause, menopause, and postmenopausal health concerns. The OB-GYN will discuss symptoms, coping strategies, and treatments. Age-related issues like osteoporosis, heart disease, and sexual health concerns are also covered.

Physical exams are recommended based on age, symptoms, and medical history. These may include pelvic exams to check the vulva, vagina, cervix, uterus, and ovaries, cervical cancer screening to collect cells for testing, and clinical breast exams to detect unusual changes.

The OB-GYN may perform additional checks like heart, lung, thyroid, and skin assessments if needed.

Gynecologists offer recommended vaccinations, including those for pregnancy. Accurate information helps patients make informed choices.

A respectful dialogue is key. Gynecologists listen, share recommendations, and discuss treatment options. Patients are encouraged to ask questions for the best possible care.

By addressing comprehensive health needs, gynecologists support overall well-being at every stage of life.

Pregnancy and women's health risk statistics for Jacksonville, Florida

The American Journal of Obstetrics and Gynecology published research on the modifiable risk factors for ectopic pregnancy. It is a condition where the fetus implants outside the uterus's prime cavity, resulting in extreme cases of pregnancy mortality. This 2022 study shows that many risk factors are associated with ectopic pregnancy, while tobacco smoking was the predominant one. Women who smoked had almost 2.02 times higher odds of ectopic pregnancy than non-smokers.

According to a health needs and assessment report by the University of Florida Health, maternal, fetal, and infant health complications are a pressing health issue for Jacksonville, Duval County, compared to the state or national numbers. In 2016, the infant mortality rate in Duval County was nearly 8.3 deaths per 1,000 live births, higher than the state's overall rate of 6.1 deaths per 1,000 live births.

Teen pregnancies, if not monitored timely, are a significant risk to the mother and the baby. The report shows that Duval County experiences a higher birth rate of about 27.3% among teens aged 15 to 19 than the state's average of 19.5%. Mothers who receive prenatal care during their first trimester are less likely to have adverse birth consequences such as low birth weight and infant mortality rates. However, the report shows that only 66% of women received prenatal care in Duval County, while the state did much better, as 78% of the mothers in Florida had access to proper care. This significant decrease in mothers receiving early prenatal care in Duval County indicates a higher need for education and services around prenatal care.

The same report shows that Duval County has a higher incidence of breast cancer, accounting for more than 134 cases per 100,000 females. On the other hand, the state has an incidence rate of 118 cases, much lower than Duval County. During 2014-2016, the County observed more than 21 deaths per 100,000 females due to breast cancer, while the state's average was about 19.8 per 100,000.
